Terms of Service Acceptance Form	This form must be signed and returned to Xensei by all new customers. It simply states that you have read and agree to Xensei's terms of service.
Credit Card Authorization Form	Use this form if you would like to pay your Xensei invoice via credit card, or have your Xensei service fees automatically charged to a credit card each month.
E-Mail Invoicing Authorization Form	Use this form if you would like to have your monthly Xensei account statement (bill) sent to you via e-mail rather than postal mail.
New Customer Referral	Using this form, you can quickly and easily refer new customers to Xensei. For each customer you refer to Xensei who signs up for service, you'll receive a credit equal to one month of their service fees toward your own account!
Account Transfer Authorization Form	Use this form if you would like to transfer ownership of your Xensei account to another person or company.
Domain Name Registration Form	Use this on-line form to register an Internet domain name through Xensei, or to transfer an existing domain name to Xensei's servers.
